On February 12, 2014, two workers were burned and some 84,000 pounds of sulfuric acid were released during an incident at the facility. Less than a month later, sulfuric acid sprayed and burned two contract workers during piping removal. Both incidents occurred in a unit where high-octane blending components are produced for gasoline.
CSB’s evaluation included a review of previous incidents, worker statements, and gaps in safety standards. The investigation found a number of safety culture concerns at the refinery.
