Illinois Manufacturer Cited for Asbestos Exposure
An Illinois display sign company is facing penalties of $1.2 million after five unprotected and untrained workers were allegedly required to conduct asbestos removal, which exposed them to the cancer-causing material.
OSHA levied 19 willful and 8 serious health citations following inspections at the company’s facilities last year. The action came in response to a referral from the Illinois Environmental Protection Agency.
Secretary of Labor Hilda Solis said that “such disregard will not be tolerated….No one should risk serious illness or death to earn a paycheck.”
Conveyor Accident Means Fine for Recycler
OSHA has cited a New York recycling company for 15 violations of safety and health standards. The enforcement action follows an incident last year in which an employee’s arm was severely lacerated when it became caught in a conveyor belt. According to OSHA, the equipment activated while the worker was trying to clear a paper jam.
The recycling facility was cited for lockout/tagout and bloodborne pathogen violations, among others.
OSHA inspectors found that the conveyor had not been de-energized and locked out as required before the attempt to clear the jam. A supervisor had not ensured that the employee was clear of the hazard before activating the machine.

OSHA also cited the company because employees had not cleaned up the worker’s blood after the accident, and those involved had not received training in bloodborne hazards.
OSHA’s Buffalo area director Arthur Dube said that risks such as the ones involved in this case “can be addressed by equipping employees with knowledge and tools to minimize work-related hazards and help them do their jobs safely.”
Fatality Results in 46 Violations
OSHA cited a grain company for 46 violations following the death of a 20-year-old employee, who was killed after becoming caught in a discharge augur while cleaning out a grain bin. The employer faces penalties totaling $465,500.
The citations included two willful violations for failing to lock out the augur and for not providing an appropriate grain bin entry permit to perform work in the confined space.
Serious citations addressed training problems, lack of an attendant during bin entry, and failure to have rescue equipment, among others.
“This tragic death could have been prevented had the grain bin owner and operators followed occupational safety standards and learned from the tragedies that have occurred in other grain bins,” said OSHA administrator Dr. David Michaels.
