Three workers were hospitalized when more than 30 pallets of filled glass bottles collapsed at a Springfield warehouse. One worker had to be rescued from under the rubble. Two of the injured were temporary workers. According to OSHA, the logistics company, which had supervisory responsibility for all three employees, failed to report the hospitalizations to the agency as required. OSHA cited the company for one willful and 10 serious violations with proposed penalties exceeding $100,000. As well, the employer was placed in OSHA’s Severe Violator Enforcement Program.
