“We were regularly investigating trench cave-ins. In fact, we investigated about eight fatal incidents in a year and a half. It was relentless. We were constantly inspecting trenches, trying to prevent the next tragedy. But through diligent efforts, we managed to reduce this number to one in the following year and a half. This experience taught me the power of simple, effective safety measures. The KISS principle—Keep It Simple, Stupid—applies even today. Prepare the excavation, prepare the hole before allowing workers to enter. It’s amazing how often basic safety principles are overlooked, leading to preventable accidents.”
This first-hand account highlights the importance of foundational safety practices, even in today’s complex industrial landscape. As the panelist noted, many fatal accidents still involve simple hazards like falls from heights, machine entanglement, and exposure to hazardous energy sources.
